Alprazolm Withdrawal Extrapyramidal Syndrome

Abstract

|
Extrapyramidal syndrome(e.g. acut e dystonia, such as torticollis, opisthotonos, oculogyric crisis and rabbit-lice syndrome) and other known withdrawal symptoms(e.g. anxiety, agitation, concentration difficulty, headache, fatigue, insomnia,
myalgia
etc)
were observed following the abrupt withdrawal of alprazolam administered in therapeutic dose(3-3.5mg/day) for about 13 weeks in a 40-year-old female. The pathophysiologic mechanism of alprazolam withdrawal extrapyramidal syndrome is thought to be
due to
an imbalance in a critical relationship between the dopaminergic and GABA-ergic activity in basal ganglia. Physicians should remain alert to the fact that extrapyramidal syndromes may occur after abrupt withdrawal of some benzodiazepines, such as
alprazolam. To prevent alprazolam withdrawal symptoms occurrence this drug should be discontinued gradually.
